Medication Therapy Management by Board Certified Pharmacist include:
- Review all medications including supplements, herbals and other health products.
- Screen for side effects/adverse effects, unnecessary medications, interactions among medications, supplements, other health products, medical conditions you may have and foods/drinks you might be taking.
- Ensure that medications are appropriate, there is no overdosing or under dosing of medications.
- Identify duplication of therapy, unnecessary therapy or additional therapy needed.
- Establish a treatment goal based on evidence based clinical practice guidelines that is specific for patient.
- Incorporate a wellness plan into treatment plan.
- Communicate and coordinate with patient’s medical providers.
- Implement safe medication use, compliance, adherence and decrease pill burden.
- Provide a personal medication list and a personal action plan at the end of a session.
