Chronic Care Management (CCM)

Our Board Certified Clinical Pharmacist Practitioners collaborate with a licensed Medicare provider to establish, implement, revise and monitor comprehensive care plan for chronic care management.Pharmacists provide chronic care management (Asthma, Atrial fibrillation, Chronic obstructive Pulmonary disease, Depression, Diabetes, Heart failure, Hypertension, Osteoporosis, etc.) via administering face to face comprehensive medication therapy review, employ medication and disease related care coordination with physician and other qualified health care provider. This encounter could facilitate coordination of care, improved quality of life, decreased risk of death, acute exacerbation/decompensation or functional decline of Medicare beneficiaries.

Medication Therapy Management by Board Certified Pharmacist include:
  • Review all medications including supplements, herbals and other health products.
  • Screen for side effects/adverse effects, unnecessary medications, interactions among medications, supplements, other health products, medical conditions you may have and foods/drinks you might be taking.
  • Ensure that medications are appropriate, there is no overdosing or under dosing of medications.
  • Identify duplication of therapy, unnecessary therapy or additional therapy needed.
  • Establish a treatment goal based on evidence based clinical practice guidelines that is specific for patient.
  • Incorporate a wellness plan into treatment plan.
  • Communicate and coordinate with patient’s medical providers.
  • Implement safe medication use, compliance, adherence and decrease pill burden.
  • Provide a personal medication list and a personal action plan at the end of a session.
